Cancellation Policy
Private Pay Case Management Policy Purpose: This policy outlines the terms for clients receiving private pay case management services. Please review carefully before signing. 1. Nature of Services Case management may include care coordination, resource referrals, advocacy, and assistance accessing community supports. These services are not psychotherapy or clinical treatment and are not billable to insurance. 2. Fees and Payment All services are private pay. Payment is due at the time of service unless other arrangements are made in writing. Case Management Session (___ minutes): $___ Advocacy/Consultation (outside session): $___ per ___ minutes Documentation/letters: $___ per hour Accepted payment methods: [credit, check, cash, HSA/FSA]. 3. Insurance and Reimbursement The provider does not bill insurance and cannot guarantee reimbursement from out-of-network or health savings plans. A receipt can be provided upon request. 4. Cancellations Cancellations must be made at least 24 hours in advance. Sessions canceled with less notice or missed without notice will be charged the full session fee, except in emergencies at the provider’s discretion. 5. Confidentiality All client information is confidential per state and federal law. Information may only be shared with your written consent, except as required by law (e.g., suspected abuse, neglect, or danger to self/others). 6. Emergencies and Safety Case management is not a crisis service. If you express intent to harm yourself or others, or disclose imminent risk, emergency services will be contacted. If in crisis, call 988, 911, or go to your nearest emergency department. 7. Client Responsibilities Clients agree to provide accurate information, update the provider of relevant changes, and pay fees in accordance with this policy. 8. Acknowledgment By signing below, you acknowledge understanding and agreement with this Private Pay Case Management Policy. You understand that services are private pay, not covered by insurance, and not a substitute for therapy or medical treatment.
